About Us

Since 2017, Cornerstone Home Health Care has been serving the seven metro counties of the Twin Cities area and smaller rural counties in Minnesota. We’ve built relationships among case managers, local businesses and families to meet the needs of our most vulnerable community members with compassion and respect. We take an individualized, person-centered approach that allows individuals to take an active role in their wellness and their lives.

Case managers trust Cornerstone because of our collaborative approach to working with each patient, their family and other care providers as a member of one team. But what really sets us apart is our staff of specially trained and devoted caregivers. We look for service providers who are patient, empathetic, supportive, resourceful and great at problem-solving and communication.

Cornerstone Home Health Care Inc. maintains a 245D license through the State of Minnesota to provide Home and Community-Based Services (HCBS) to people with disabilities, those with brain injuries and adults over age 65.

Our Team

Our leaders have been in the home healthcare service industry for over 10 years. We know from experience that building strong personal connections with mutual trust and respect is the best way to make sure each client is getting the assistance they need.

You’ll meet a representative from our intake team during an initial session in your own home. You or your loved one will be spending a lot of time with your caregiver, so finding a good match is our top priority. There’s nothing more rewarding than thank-you calls from clients who have made it through tough situations with the help of their Cornerstone caregiver!

Mission

Cornerstone Home Health Care believes that health care is a basic human right. It must be available, coordinated and provided in a comprehensive way, combined with other human services when appropriate. We believe home health care is an important part of the continuous health care system, and it shall be provided in the most cost-effective way possible.

We believe the services should be tailored to meet the needs and wishes of the clients we serve.

Person-Centered Approach

Person-centered practices are based on the fundamental principle that government and service providers must listen to individuals about what is important to them to create or maintain a life they enjoy in the community. We see people as unique and whole individuals with potential and gifts to share, and we look at services and supports in the context of what it takes for each client to have the life they want on their terms.

Positive Supports

Positive supports are strategies used to increase quality of life and decrease challenging behavior. We use strategies that encourage our clients to use productive, alternative ways to deal with stress. We will never engage in procedures that restrain, isolate or punish individuals in our care or otherwise constitute aversion, deprivation or abuse.
